Have you tried to break it down bit by bit so it won't overwhelm you?
Rather ask for help for everything which can be daunting look at and prioritise what needs to be looked at first in relation to yourself.
I can relate to how debilitating panic attacks can be. I look at the most important thing that needs to be looked at and what strategies to put in place.

I have been trying to ask for help carers aust but i cant seem to ring them. my anxiety is really bad and cant even dial the number now. every time i just get my anxiety back to a manageable amount i think i could try again but then i panic all over again. im to scared to even speak to anyone in the family and with no friends i offline i dont know what to do. its really hard for me. i dont really know how to take these steps forward when it feels like i have to get permission or something. i dont understand why im so afraid

I feel for you. It can be very daunting not knowing how a family member will react to you.
However you have done so well on the forums to express what is challenging you. It can be hard to take the next step.
From what you have expressed here on the forum it sounds as if your family would actually understand if you need to take action. Your family need to know how crippling it is for you.
Perhaps you would like someone to mediate for you, perhaps a mental health professional to help ease your way in to the conversation and discuss what the challenges are and how they can be overcome and to step forward not only for you but for your family as well. To create a better health and wellbeing for all, including you.
Would it help for you to type/write it down on a piece of paper what your thoughts, feelings and challenges are with what you are dealing with and ask someone in your family to read it so that way if you find it difficult or challenging to speak up, someone can read your wishes?

Perhaps writing down what you need to say might help sweetie. Just take it one step at a time.

You have so much going on @outlander at the moment and it probably feels very overwhelming for you. No the panic attacks would have not helped today.
